Class-Act Contractor Availability Update Form

This form is for contractors to provide notice of temporary changes in their ability to perform contracted work. Submissions are for visibility only and do not create or imply any right to time off, benefits, or guaranteed assignments. To ensure proper coverage of client commitments, CASP strongly prefers that any changes be submitted at least seven (7) days in advance. Changes will only be recognized once confirmed by the CASP office. You will receive an email confirmation upon submission, followed by either a call, text notification, or additional email once the office has reviewed your notice.
